Player Injuries

Lacombe Minor Hockey Association has Return to Play Protocol for injured players.

When a player is injured please fill out the Injury Reporting Form that goes directly to our Lacombe Minor Hockey Safety Officer. She will follow up and go over any next steps.

If a player is injured during a team sanctioned event then a Hockey Alberta injury form must be filled out and returned to your coach.

Once a player is ready to return to play, a doctors note approving the return must be given to the coach prior to participation
